OUR APPROACH

COLLECTIVE IMPACT

The Health Equity Collective adopted the Collective Impact Model in developing a systemic approach to promoting health equity across Greater Houston as a part of its 2019 visioning and formation. Grounded in Kania and Kramer’s five phases of collective impact for coalition building, the 140+ organization Coalition operationalizes its mission through its backbone leads, steering committees, and eight workgroups; each with a goal that mutually reinforces and advances the Collective’s vision. To date, phase one (generating ideas), phase two (initiating action), and phase three (organizing for impact) have been completed. Phases four (implementation) and five (sustainability) are currently underway.

STRUCTURE & ROLES

The Health Equity Collective’s membership includes organizations working across the spectrum of upstream to downstream health equity interventions, social determinants of health (SDOH), and health factors. The Collective actively looks to align and strengthen existing efforts within the community and connect silos of work and silos of data for greater impact and improvement in our region. We know that the key to achieving the changes we want to see for our communities is our relationships, and that strength is based on trust. The Health Equity Collective’s priority work areas are organized into workgroups with 15-45 members contributing towards annual goals which align with the Collective’s priorities.
