History

Our Evolution

The Health Equity Collective launched in late 2018 to bring together passionate people, including those in the healthcare and social service institutions that serve the region, who are ready for a systemic, population-level approach for addressing SDOH. Follow our evolution below.

2021

This collective impact effort transitioned to the action phase. Click below to view our resources.

2020

With the common agenda, shared vision & mission established, 2020 was focused on developing the coalition’s structure. The workgroups were launched and successfully implemented plans, even through the pandemic.

2019

The backbone team conducted a listening tour, facilitated a common agenda for the shared effort moving forward, and adopted the collective impact model to guide the alignment of SDOH efforts across our community.

2018

The Clinton Health Matters Initiative effort wrapped up in Houston. The American Heart Association, Harris County Public Health, and UTHealth agreed to continue the work to address SDOH.
